If you would like a "fill-in-the-blanks" letter ruling request template that you may use in Word to generate a letter ruling request, please right click on the following and choose "Save Target As" to save the template on your computer (NOTE: If using Internet Explorer, when the dialog box "Save File As" opens, you will need to change the file name to "Letter Ruling Request Template.dot"). Use your "tab" button to move to the next field:

You may then open it on your computer, fill in the blanks with the pertinent information, print and mail it to the Department of Revenue. DOR must respond to properly completed letter ruling requests within 60 days.
